  "description": "**Concept**\\r\\nAn alert describes a clinical or administrative fact brought to the attention of the users of the clinical systems to be taken into account when shaping diagnostic and therapeutic policy or in dealing with the patient, usually because of a safety risk. \\r\\nDisorders that describe the body’s sensitivity to a substance which results in a specific physiological reaction after being exposed to that substance are referred to as allergies. These are described in a separate information model. \\r\\nWarnings for non-allergic disorders can concern: \\r\\n\\r\\n* A disorder, condition or diagnosis which can be considered as a contraindication for undergoing a certain type of therapy, such as pregnancy or long QT syndrome; \\r\\n\\r\\n* Impaired functioning of an organ system (heart failure, impaired liver or kidney function, weakened immune system); \\r\\n\\r\\n* Risk of spreading certain microorganisms (multi-resistant bacteria, tubercle bacilli, HIV, HBV, Ebola virus); \\r\\n\\r\\n* Other risks \\r\\n\\r\\n\\r\\n**Purpose**\\r\\nDocumenting and entering disorders or conditions that require attention is an important part of medical registration. It concerns the core of patient safety. In the execution of research and treatment, these patient characteristics - which are marked as a warning - constantly have to be taken into account. They provide information that is important for the patient’s condition and the options a healthcare provider has for therapy. Disorders that are registered or transferred as an Alert can also be described as a Problem. The difference is in the fact that the healthcare provider considers the problem as an Alert = warning. In many cases, transfer will be subject to strict privacy rules, as the warning will not always elicit an adequate reaction in the informed environment.\\r\\n**Example Instances**\\r\\n\\r\\n**Instructions**\\r\\nIf there is a contraindication which is also important for medication safety, it should also be registered via the g-standard (Thesaurus 40).  See further HCIM MedicationContraIndication.",

Deze worden in een aparte bouwsteen beschreven\r\nWaarschuwingen voor niet allergische aandoeningen kunnen betreffen: \r\n\r\n* Een aandoening, conditie of diagnose die beschouwd kan worden als contra-indicatie voor het ondergaan van een bepaalde therapie, zoals zwangerschap of een verlengd QT-syndroom; \r\n\r\n* Verminderde functie van een orgaansysteem (hartfalen, verminder lever- of nierfunctie, verminderde afweer); \r\n\r\n* Kans op verspreiding van bepaalde micro-organismen (multiresistente bacterie, tuberkelbacterie, HIV, HBV, Ebola virus); \r\n\r\n* Andere risico’s. \r\n\r\n\r\n**Purpose**\r\nHet vastleggen en doorgeven van aandoeningen of condities die aandacht behoeven, is een belangrijk onderdeel van de medische registratie. Het raakt de kern van patiëntveiligheid. In de uitvoering van onderzoek en behandeling moet veelal continu rekening worden gehouden met deze, als waarschuwing gemarkeerde, patiëntkenmerken. Ze verschaffen informatie die belangrijk is in relatie met de conditie van de patiënt en de opties die een zorgverlener heeft voor therapie. Aandoeningen die als Alert worden geregistreerd of overgedragen, kunnen ook als Probleem worden beschreven. Het verschil is hierin gelegen, dat de zorgverlener het probleem heeft aangemerkt als Alert = waarschuwing.
